We would like to thank you for choosing us to care for your child(ren). When coming for your first visit we ask that you come 15 minutes early to fill out paperwork. We would also ask that you bring a copy of immunizations and any information that you would have from your previous provider. This information is helpful to our staff and the providers to make sure we provide you with the best care you deserve.
We have two waiting rooms. We have a waiting room for anyone who is sick and a waiting room for those who are here for well-child visits and non-illness related appointments. Please let the front desk know if you are all well and wish to go to the Well Waiting room.
